Trainer Info
It is mandatory to fill in the information for trainer. The USEF definition of trainer is the person who feeds and cares for your horse during the event. It is not the the person who gives you lessons! The Trainer is required to have a USEF # at the Preliminary level and above.
Make sure your entry form is complete! Incomplete entries are moved to the back of the draw pile, even if they are postmarked opening day.
What do we mean by complete entry?
Must be legible.
One entry per horse, with separate entry fee checks (if you enter 3 horses, there should be 3 entry forms and 3 entry fee checks. DO NOT COMBINE multiple entry fees on one check.).
USEA, USEF, FEI numbers for riders, horses, and owners MUST be filled in where applicable.
ALL required signatures are present (Rider, Owner, Trainer, Guardian if applicable).
Please fill in rider's birth date (used to determine eligibility for Junior, Young Rider, Masters divisions).
Copy of current Coggins.
If for some reason you are missing information (ex. you just bought a new horse and it doesn't have a number yet) include a note explaining the situation. When you do get the missing information, call the secretary immediately so we can update your entry.
